Most people looking for the cause of their hearing loss are looking for one clear explanation. They wonder if it was the decades spent around machinery. The concerts, hunting, or power tools that filled their lives. Maybe hearing problems run in the family. Or maybe there’s no obvious explanation at all. You’ve just realized that hearing takes more effort than it used to.
Pinpointing the cause of hearing loss isn’t always as straightforward as identifying one event or one risk factor. Aging and noise exposure are common contributors, but hearing can also change because of earwax, middle ear problems, medications, illness, injury, genetics, or conditions affecting the inner ear and auditory nerve. Sometimes, more than one factor is involved. Understanding what may be causing the change starts with identifying the type of hearing loss.
Hearing depends on several parts of the auditory system working together. Sound must travel through the ear canal and middle ear before reaching the inner ear, where it is converted into signals that travel to the brain.
A problem anywhere along that pathway can affect hearing. Common causes of hearing loss include aging, repeated noise exposure, earwax, infections, certain medications, genetics, injuries, and conditions affecting the inner ear or auditory nerve.
The location of the problem generally determines whether hearing loss is classified as conductive, sensorineural, or mixed.
Conductive vs. Sensorineural Hearing Loss
Conductive hearing loss occurs when something interferes with sound traveling through the outer or middle ear. Depending on the cause, it may be temporary or medically treatable.
Sensorineural hearing loss results from damage to the inner ear or auditory nerve. Age- and noise-related hearing loss fall into this category and are generally permanent.
Mixed hearing loss means both types are present at the same time.
This distinction matters because two people who both describe their hearing as “muffled” may need very different care.

Aging and Genetics
Age-related hearing loss typically develops gradually and often affects both ears. Because the change can happen slowly, family members may notice communication difficulties before the person experiencing them does.
Genetics can also influence hearing loss, including how susceptible someone is to certain hearing conditions.
Noise Exposure
Noise-related damage can occur after an extremely loud event or build over years of repeated exposure. Machinery, power tools, firearms, motorcycles, concerts, and loud recreational activities can all contribute.
This is particularly relevant for people who have spent years around loud equipment at work or participate in activities such as hunting. Hearing may seem adequate in quiet settings, while conversations become increasingly difficult to follow in background noise.
Ototoxic Medications
Certain medications can damage structures involved in hearing and balance. These are known as ototoxic medications and include some chemotherapy drugs, antibiotics, and other treatments.
Patients concerned about a medication should talk with the prescribing medical provider rather than stopping treatment on their own.
Illness, Injury, and Inner Ear Conditions
Some infections and illnesses can damage the inner ear. Head or ear injuries can also affect hearing.
Inner ear disorders such as Ménière's disease may involve hearing changes along with symptoms such as tinnitus, ear fullness, or vertigo. Autoimmune conditions can sometimes affect inner ear function as well.
Auditory Nerve Disorders
Less commonly, hearing difficulties can involve the nerve pathways carrying information from the inner ear toward the brain. Auditory nerve disorders and certain tumors may cause hearing changes, particularly when symptoms are noticeably different between the two ears.
Mixed hearing loss occurs when a person has both conductive and sensorineural hearing loss.
For example, someone may already have permanent age-related inner ear damage and later develop enough earwax or middle ear fluid to further reduce hearing. Addressing the conductive problem may improve hearing, but it does not eliminate the underlying sensorineural loss.
One-sided hearing loss has many possible explanations, including earwax, infection, fluid, eardrum problems, injury, sudden sensorineural hearing loss, and less-common conditions affecting the inner ear or auditory nerve.
The important point is that one ear being worse than the other is useful diagnostic information. Rather than assuming the ear is simply blocked, an evaluation can help determine where the hearing change is occurring and whether additional medical assessment is appropriate.

When to Seek Medical Care
A sudden drop in hearing should be treated differently from hearing loss that has gradually developed over several years.
Sudden sensorineural hearing loss can occur rapidly and is sometimes mistaken for congestion, earwax, or an ear that needs to pop. Because timely treatment can matter, a sudden unexplained hearing change, particularly in one ear, should receive prompt medical attention rather than waiting to see whether it clears on its own.
Symptoms alone usually cannot identify the cause. “My hearing is muffled,” “everyone mumbles,” and “I can hear people but can't understand them” might describe your real problems, but they do not reveal whether the issue involves the ear canal, middle ear, inner ear, or another part of the auditory system.
